Iizukatei is the strongest recommendation if you want a calm countryside stay centered on heritage architecture, local food, and a slower town-based experience away from Nikko’s busy core. Isamikan is the better alternative if you specifically want a rural onsen inn with river views and a more traditional small-ryokan feel. Iizukatei is not a true onsen-first stay, while Isamikan is weaker for access and mobility needs.
Choose Iizukatei for heritage and local culture; choose Isamikan if hot spring bathing matters more than design and access ease.
